Based in our Dubai office the chief responsibility of the Care Coordinator is managing client communications interacting directly with potential, new and existing VIP clients. You will be required to provide quality customer service, while increasing the company’s revenues through increased sales and client satisfaction. As a member of the management team, make a constructive and effective contribution to the formulation of The Health Bank’s key business and service objectives and formulate and progress SOP’s that allow the company to achieve those objectives This is a challenging role, involving highly demanding situations to ensure that all the clients’ requests relating to their health, wellness and care management are met. Your professional skills and knowledge within a healthcare domain will be critical in understanding and enabling a pro-active and caring client centric approach.
The Care Coordinator will perform the following tasks:
· Handling enquiries
· Answering incoming calls
· Offering clients products and services
· Performing administrative functions
· Updating client accounts
· Management of medical records, both digital and manual formats
· Maintenance of client, medical providers and emergency contact databases
· Implementation, design and review of SOP’s/Protocols relating to business services
· Liaison and follow-up with external care providers/organizations
· Processing customer correspondence
· The Care Coordinator at times may work over the evening and weekend and will be part of covering the 24 Hours on Call on per Rota basis.
· Responsible for their own time management with members.
· Attending appointment with member’s.
· Managing member caseloads that are assigned to the Care Coordinator.
· Serve as a contact point, advocate and informational resource for the member and/or the family
· Promote timely access to appropriate care.
· Connecting member to relevant care providers, with the goal of enhancing patient health and well-being.
· Facilitate patient access to appropriate medical and specialty providers Globally.
· Increasing members ability for self-management and shared decision making.
· Adhering and supporting the member to follow up plans by recommended by the care provider or various professionals.
· Provide support to the member during life transition, surgery, chronic illness etc.
· Stay available to the member/family at the time of urgent matter.
· Supporting continuity of care.
· Facilitating members unmet health needs.
· Developing and creating a health management plan and reviewing with the member/ family/ guardian.
· Participating in weekly team handover meetings.
· Acknowledges patient right to Confidentiality and maintain confidentially always. Adheres to company and international laws.
· Researching and providing educational material relevant to the members health and wellness requirements.
· Responsible for the up keep of members Electronic Medical Record and legal documentation.
· Escalating any complaints to your line manager.
· Highly organized and being able to keep accurate notes and records.
· Ability to work as part of a team and use of own initiative.
· Responsible for keeping self-up to date of latest Clinical knowledge and medical trends.
· Attend relevant training to the role.
· Adaptable to changes/business decisions.
