الوصف الوظيفي Job Description: Performs evaluations and develops effective Speech Language Pathology treatment plans to restore, maintain or prevent decline of patient function, by planning and administering medically prescribed therapy treatments in accordance with professional standards. The SLP must also be able to set realistic and achievable goals for their patients as well as document and record the patient’s condition using the correct documentation. Performs required duties at a high level, with minimal orientation. Remains flexible regarding work schedule to meet client‐ staffing requirements. Job Knowledge / Responsibilities: General Duties · Provides patient care as a Speech Language Pathologist to patients. · Evaluates patient’s speech and language abilities and level of function · Develops treatment plans, including short and long‐term goals and methods to achieve identified goals · Provides treatment as ordered by the attending physician · Communicating the patient’s speech therapy program to the family and/or guardian · Submits written documentation including evaluations, daily notes, weekly summaries and discharge summaries for hospital records · Perform related duties and responsibilities as required. Documentation · Properly and accurately documents/charts observations and other data related to the clinical condition of the patient. Age Specific · Delivers age appropriate care for the patient population. Continuing Education · Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. · Attends and participates in continuing education programs to monitor proficiency, expand current knowledge and enhance professional excellence. Willingly shares knowledge and provides clinical orientation to other members of the staff. Confidentiality · Preserves and protects patient and client confidentiality in all situations and with all documentation.
