الوصف الوظيفي Coordinates the workup of transplant recipients and donors, pre and post-transplant patient care for organ recipients and donors. Performs a variety of tasks in clinical settings in collaboration with medical and nursing staff and accountable for the assessment, planning, implementation and evaluation of care to specific patient population. Essential Responsibilities and Duties 1. Coordinates solid organ transplantation with patient, donors, families, medical staff, organ donor coordinators and administrative staff. 2. Provides appropriate follow-up care to recipients and donors ensuring all relevant information is available to enable patients to make informed decisions and choices concerning their treatment options. 3. Maintains and regularly reviews clinical status of patients on the waiting list for donation and ensure all diagnostic investigations and results are up to date; ensuring logistical arrangements are in place to facilitate emergency admission for transplant. 4. Coordinates care of potential organ transplant patients, potential donors, transplant candidate evaluation process, organizes organ procurement and transplant discharge planning and follows post-transplant care. 5. Functions as a specialist nurse to provide support and education to staff and patients in the specific speciality areas. 6. Organizes and participate multi-disciplinary team conferences, rounds or meetings related to care of transplant patient or program issues. 7. Participate in 24-hour transplant and organ coordinator on-call rotation. 8. Coordinates and documents patient care utilizing the nursing care plan, medical records, teaching material and patient teaching records. 9. Documents all relevant information correctly, appropriately and in a timely manner. 10. Documents all relevant information correctly, appropriately and in a timely manner. 11. Develops policies, procedures and protocols for transplantation and assists in implementation as required. 12. Develops and coordinates educational programs for transplant patients, donors, families and nursing staff in conjunction with Nursing Education and research personnel. 13. Provides assessment, plan of care and evaluation of specific patient population and provides follow-up care as necessary. 14. Liaises with physicians and other transplant healthcare staff within the division, the hospital and other healthcare institutions as directed by Director, Organ Transplant Center and supervised by the Transplant Coordination Team Leader. 15. Participates in all mandatory departmental activities and the Organ Transplant Centre’s Activities, i.e. Staff Meeting, Core Lectures, Research Forum, etc. and serves on departmental and hospital committees, as required. 16. Participating and contributing to the research activities of the centre including involvement in data collection, dissemination and reporting. 17. Actively contributes to the Division’s and/or the Department’s documentation of patient care processes and outcomes in the medical chart and/or The Organ Transplant Registry. 18. Upholds the organizational values at all times. 19. Participating in the professional development and training of clinical transplant coordinators. 20. Following and monitoring highest standards of professional and ethical codes of conduct. 21. Complying with all related policies and procedures. 22. Follows all Hospital related policies and procedures. 23. Participates in self and other’s education, training and development, as applicable. Education Bachelor’s Degree in Nursing or Registered Nurse with country specific post registration program. Current Registered Nurse licensure from country of origin required. Experience Required Three (3) years of hospital experience in acute care setting including one (1) year in critical care setting or in transplant required.
